Thank you for taking the time to complete the evaluation of the Global Health Council’s 32nd Annual Conference. The Council depends on your input to help us improve the conference from year to year. Your opinions, comments and suggestions are important to us.
Please complete the general conference evaluation and a session evaluation for each session that you attended (workshop, panel, and/or roundtable):
1: General Conference Evaluation
Have you previously completed and turned in a paper copy of the general evaluation form for this conference?
If so, thank you! You will be entered into the drawing for a free registration.
If not, please complete the general conference evaluation.
After the evaluation has been submitted, you will be asked to enter your name and e-mail address in order to be entered into the drawing for the free registration to the 2006 Annual Conference. Entry into the drawing is optional. Only those who complete the general conference evaluation form, either the paper copy or the online form, by June 30, 2005 will be entered into the drawing.
2: Specific Workshop Session Evaluation
Complete the workshop session evaluation.
3: Specific Panel Session Evaluation
Complete the panel session evaluation.
4: Roundtable Session Evaluation
Complete the roundtable session evaluation.
